To: Public Information Officer, Arizona Health Care Cost Containment System (AHCCCS)Dear Public Information Officer,Pursuant to the Arizona Public Records Law (A.R.S. § 39-121 et seq.), I am writing on behalf of a non-profit organization to request copies of the following public records maintained by the Arizona Health Care Cost Containment System (AHCCCS).To ensure clarity and prevent administrative delay, please note that this request is strictly limited to aggregated, anonymized, system-wide administrative data. We are not requesting, and explicitly instruct the agency to exclude, any Personally Identifiable Information (PII) or Protected Health Information (PHI) subject to HIPAA redactions.

Requested Records:

  1. Application Processing Volume: Monthly aggregated totals of new applications received for Home and Community-Based Services (HCBS) long-term care waivers from January 1, 2025, through the date of processing.
  2. Approval & Denial Metrics: Monthly aggregated totals of determinations made on these applications, broken down specifically by:
    • Total number of approvals.
    • Total number of administrative or technical denials.
  3. Denial Code Breakdown: A system-generated list of the specific denial or reason codes associated with the administrative/technical denials identified in Item 2, alongside the aggregated frequency of each code per month.

Non-Commercial Purpose Certification: Pursuant to A.R.S. § 39-121.01(D)(1), I hereby affirm that these records are requested solely for a non-commercial purpose. These metrics are being gathered by a 501(c)(3) public charity for nonpartisan data analysis, system transparency mapping, and public education. They will not be sold, used to solicit business, or utilized for monetary gain.Format of Production: As these are electronic, database-driven tracking metrics, please provide these records in their native electronic format (e.g., as an exported .csv, Excel spreadsheet, or digital PDF) via return email to minimize administrative costs.
