Pursuant to the Freedom of Information Act (5 U.S.C. § 552), I hereby request electronic copies of existing datasets and agency records managed by the Center for Medicaid and CHIP Services (CMCS) regarding monthly state data submissions for the States of Arkansas (AR), Indiana (IN), and Ohio (OH) from June 1, 2025, through the present. Specifically, I request the following: 1. T-MSIS ANALYTIC FILES (TAF): The monthly aggregate file extracts for the specified states and date range, specifically targeting the TAF Demographic and Eligibility (TAF_DE) files and the TAF Other Services (TAF_OT) files containing service delivery metrics, capitation metrics, and authorized service hour data for Home and Community-Based Services (HCBS) and 1915(c) waiver enrollment categories. 2. T-MSIS DATA QUALITY (DQ) OUTPUTS: All processing logs, error notifications, and Data Quality validation reports generated via the CMS Outcomes-Based Assessment (OBA) framework or the T-MSIS Operations Dashboard concerning reporting anomalies, completeness degradation, or data cessation for the specified states from January 1, 2026, through the present. 3. STATE CORRESPONDENCE REGARDING REPORTING RESTRICTIONS: All formal communications, deficiency alerts, or administrative updates between CMS regional offices and the respective state Medicaid agencies (Arkansas DMS, Indiana FSSA, Ohio ODM) regarding changes to, implementation of, or data drops associated with independent assessment tools, algorithmic care-hour formulas, or resource utilization groupings.
